Complete/Submit the LAUSD online Volunteer Application (print, sign, and submit to office) - Signed Volunteer Commitment and COVID-19 Waiver Forms
- Tuberculosis (TB) Test Clearance (completed within 60 days of submitting online volunteer application)
- Proof of COVID-19 Vaccination (submit to school and upload to LAUSD Daily Pass within 45 Days of submitting online application)

steps to becoming a LAUSD Approved volunteer
Once the application is completed, the application must be printed, signed, and submitted to the school along with the signed Volunteer Commitment Form, Tuberculosis Test Clearance, AND Proof of COVID-19 Vaccination. The school must review (ensure required fields are completed).
All forms & documents may be submitted to the Topeka Main Office or the Community Representative, Kelly Paguio: kelly.paguio@lausd.net
- Online application - printed and signed
- Volunteer Commitment Form – printed and signed
- COVID-19 Waiver of Liability Form - printed and signed
- Tuberculosis (TB) Test Clearance
- Proof of COVID-19 Vaccination (uploaded to the Daily Pass)
*Please note: On-campus volunteers, in addition to being cleared through the online CA Megan’s Law website, must submit a Daily Pass each day they will be on campus. For assistance, contact your school site or the Los Angeles Unified Family Hotline at 213-443-1300.
All VOLUNTEERS must be approved by LAUSD prior to volunteering on campus.
